Integrative Psychiatry and Wellness — IPW Health — is a psychiatric and therapy practice founded by Shahrzad Shadmani, PMHNP-BC, FNP-C. It was built around one observation from years of clinical work: mental health symptoms rarely have a single cause, and treating them without looking at sleep, hormones, thyroid and metabolic health leaves a lot of people only partly better.
The practice provides psychiatric evaluation, medication management, and therapy — individual, couples, and EMDR for trauma — so that care does not have to be assembled from separate providers who never speak to one another.
In-person appointments are available in Mission Viejo, California, with Shahrzad, who is the practice’s only clinician seeing patients in person. All other care is delivered by secure telehealth across California, Oregon, Washington and Colorado, by a team of psychiatric nurse practitioners and licensed therapists.
See Insurance & Pricing for accepted plans, or meet the team.
Care begins with a full evaluation rather than a symptom checklist: what is happening now, what has already been tried, and the physical contributors that often sit underneath psychiatric symptoms — sleep quality, thyroid function, hormonal change, and metabolic health.
Where medication is the right tool it is used deliberately and reviewed over time. Where it is not, therapy or a different approach is the better answer. Both are available here.

Available for in-person visits at our Mission Viejo office or by secure telehealth.
I am Shahrzad Shadmani, a dual board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) and Family Nurse Practitioner (FNP-C), and the founder of Integrative Psychiatry and Wellness. I have worked in healthcare since 2007, the last four years focused on mental health, and I am licensed in California, Oregon, Washington and Colorado.
I trained in both psychiatry and primary care because the two questions are rarely separate. Fatigue, poor concentration and low mood are produced by thyroid disease, iron deficiency, B12 deficiency and untreated sleep apnea as readily as by depression — and in a psychiatry-only practice that half of the question gets referred out and waits. I order and interpret laboratory work here, so it does not.
What to expect from a first appointment: a full evaluation rather than a fifteen-minute prescription visit, a diagnostic impression stated plainly, and a clear account of what I am still unsure about and what would settle it. If what you need is therapy rather than medication, I will say so.
This practice aims to bridge the gap between medical and mental health services, offering focused, individualized, and customized care that looks at the biological, psychological, and lifestyle factors contributing to our clients’ concerns. We are committed to ensuring that each person’s needs are met as a whole, considering their physical, emotional, and psychological well-being.
PMHNP-BC — Psychiatric Mental Health Nurse Practitioner across the lifespan, certified by the American Nurses Credentialing Center.
FNP-C — Family Nurse Practitioner, certified by the American Academy of Nurse Practitioners Certification Board.
Both certifications are national, examination-based, and must be renewed on a fixed cycle with documented continuing education and clinical practice hours. Holding both is uncommon.
Psychiatric training covers diagnosis and medication management. Family practice training covers the medical conditions that imitate psychiatric ones — thyroid dysfunction, anemia and iron deficiency, vitamin B12 deficiency, disordered glucose metabolism, and obstructive sleep apnea, which is substantially underdiagnosed in women and routinely mistaken for depression.
In practice that means laboratory work is ordered and interpreted here rather than handed off, and that the metabolic monitoring several psychiatric medications require actually happens. It also means unvalidated testing is not used: no broad “functional” panels, no hair mineral analysis, no IgG food sensitivity testing.
Master of Science in Nursing and post-master's certificate, Purdue University, completed 2021 — with graduate preparation as both a Family Nurse Practitioner and a Psychiatric Mental Health Nurse Practitioner across the lifespan.
Before that, a Bachelor of Science in Nursing in Texas in 2016, and entry to nursing practice at Cerritos College in Norwalk, California, in 2012.
That career began at the bedside — Hoag Hospital in Newport Beach, Mission Hospital in Mission Viejo, and travel nursing assignments including Torrance — across a range of settings from emergency care to on-call work. Mental health was part of the caseload in every one of them, and that is what eventually directed her training toward psychiatry.
The starting point was earlier still. In 2007, before nursing school, Shahrzad certified through after-school training programs to work in long-term residential homes for children with disabilities. That work is where the rest of this came from — the conviction that caring for people through what they cannot manage alone was the work she was meant to do.
In healthcare since 2007, with the last four focused on psychiatric and mental health care. Licensed as an advanced practice nurse in four states, each with its own board review and renewal requirements.

Shahrzad Shadmani, MSN, APRN, PMHNP-BC, FNP-C is dual board-certified as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC, ANCC) and as a Family Nurse Practitioner (FNP-C, AANP), and is licensed to practice in California, Oregon, Washington and Colorado. She is the founder of Integrative Psychiatry and Wellness.
The dual certification is unusual, and it is the reason this practice is built the way it is. Psychiatric training covers diagnosis and medication management across the lifespan. Family practice training covers the medical half of the same question — thyroid function, iron status and anemia, vitamin B12, glucose and lipids, sleep-disordered breathing, and the long list of ordinary medical conditions that produce fatigue, poor concentration and low mood indistinguishable from a depressive episode. Most psychiatric practices refer that half out and wait. Here it is examined in the same evaluation, because she orders and interprets laboratory work directly.
Depression and treatment-resistant depression; generalized anxiety, panic and social anxiety; obsessive-compulsive disorder; post-traumatic stress disorder; bipolar disorder; adult ADHD, including full diagnostic evaluation; premenstrual dysphoric disorder and mood symptoms across the perimenopausal transition; insomnia and circadian disruption; and the mood and cognitive changes that accompany hormonal, thyroid and metabolic disorders.
A first appointment is a full psychiatric evaluation, not a fifteen-minute prescription visit. You leave with a diagnostic impression stated plainly, a treatment plan that names what is being treated and how, and an explicit account of what is still uncertain and what would change it. Where a first-line treatment is psychotherapy rather than medication, that is what gets recommended.
Medication is prescribed on the same evidence base as anywhere else, and monitored — including the metabolic monitoring that several psychiatric medications require and frequently do not get. Stimulant and non-stimulant medication for ADHD can be prescribed by telehealth in all four states served. Unvalidated testing — broad “functional” panels, hair mineral analysis, IgG food sensitivity testing — is not used here, and supplements are not substituted for treatment that works.
