San Juan Capistrano holds on to its history: the Mission bells, the old adobes along Los Rios Street, the trains rolling past the Capistrano Depot, and the horse trails that still run through town. Life here can feel unhurried, but trauma, anxiety, intrusive thoughts, grief or a long stretch of low mood don’t care about the pace of a place. Integrative Psychiatry & Wellness (IPW Health) offers psychiatric care and therapy for San Juan Capistrano residents, with same-week appointments for new patients and a choice of in-person or secure video visits.
Our office is at 27201 Puerta Real, Suite 300, in Mission Viejo, off Crown Valley Parkway. From San Juan Capistrano City Hall on Paseo Adelanto, it’s about 6 miles: take Del Obispo Street or Ortega Highway to the I-5 north, exit at Crown Valley Parkway and head east. In light traffic that’s typically 10 to 15 minutes. From San Juan Hills or the newer neighborhoods off La Pata, Antonio Parkway to Crown Valley Parkway is an easy alternative. If you ride Metrolink, the Laguna Niguel/Mission Viejo station is the next stop north of San Juan Capistrano and about 2 miles from our office.

In crisis or thinking about harming yourself? Call or text 988, or call 911. IPW Health is an outpatient practice, not an emergency service. More options are on our crisis resources page.
Our licensed therapists see California clients by secure video, and each focuses on approaches with solid evidence behind them. EMDR and trauma-focused therapy can help when a past event keeps intruding on the present, whether that’s a car accident on the 5, a medical crisis or an experience from childhood. Exposure and response prevention (ERP) is a first-line therapy for OCD, and doing it from home can actually help, because many exposures are practiced where symptoms show up. Grief counseling offers room to process a loss at your own pace.
Not sure which approach fits? Our article comparing EMDR and CBT for trauma explains the differences in plain language.
Sometimes therapy alone is enough. Other times it helps to add a psychiatric evaluation, which looks at your symptoms, history, sleep, health and goals. An ADHD evaluation can clarify long-standing trouble with focus and follow-through. When clinically appropriate, medication management with regular follow-up can be part of your plan and coordinated with your therapist. We also look for physical contributors such as thyroid problems, low iron and hormonal changes.
IPW Health was founded by Shahrzad (Shery) Shadmani, PMHNP-BC, FNP-C, a psychiatric nurse practitioner who is also board-certified in family practice. In-person psychiatric visits at the Mission Viejo office are with our founder, Shahrzad (Shery) Shadmani, PMHNP-BC, FNP-C, on Mondays, and in-person therapy, including couples therapy, is offered there on Saturdays. Psychiatric nurse practitioners Merrick Tsai and Beverly Glavan see patients by secure telehealth, and telehealth visits are available throughout the week. Many San Juan Capistrano patients mix the two: an in-person evaluation, then follow-up by video when life gets busy. Our guide to in-person vs. telehealth care can help you decide.
Psychiatric telehealth covers California, Oregon, Washington and Colorado. Therapy is for clients located in California. Visits are offered in English and Farsi, including therapy in Farsi. We also see patients from neighboring San Clemente and Ladera Ranch.
Insurance and cost: We are in network with Aetna, Anthem, Blue Cross Blue Shield, Cigna, Medicare, Optum and UnitedHealthcare. Coverage varies by plan, so we verify your benefits before your first visit. Self-pay is available too: a psychiatric evaluation is $350, a follow-up visit is $200, and therapy sessions range from $140 to $250. Self-pay patients receive a Good Faith Estimate before the first visit. See plans and pricing.
